Hole in the back and a few screws, is all. :)

Unless the cabinet is fully enclosed, as long as you have adequate ventilation above the amp, it should be OK, Clive.  It's not as though you have the amp on all the time.  I only have mine on now when watching a DVD, as the TV sound is more than adequate for normal programmes.  One thing you could do to increase ventilation, is remove the back from the cabinet, if it has one.  Mine is open backed, and I have no problems with over heating.  :)

Yup. Go for a large fan, they tend to be quieter. Simon is right that an open backed cabinet will help, but forced ventilation is often more effective, and one of the biggest causes of failure is over-heating. Remember the power amp will be dissipating heat in proportion to the sound level.
